CONFIDENTIALITY NOTICE
Confidentiality protects the client's privacy by law.
There are however exemptions where the counsellor may have to disclose to a third party.
These include:
the counsellor has reasonable grounds to assume that the client may harm themselves or others, or
the client is disclosing to the counsellor that he or she is committing a serious crime, or has
committed a serious crime in the past.
the counsellor has mandatory reporting duty in the event of child abuse, or
the counsellor has reasonable grounds to assume child abuse.
a child is person under 16 years.
CONSENT NOTICE
I have read the above information carefully and
I understand and accept the terms and conditions under which services will be provided to me.
if anything about my situation or circumstances changes in a way which will impact on the effectiveness of counselling,
I will immediately inform my counsellor.
I understand that a person’s reaction to counselling can vary and that, occasionally, despite best practices,
an adverse outcome is achieved.
I release my counsellor, Heide McConkey, from liability for any damage, which I sustain as a result of counselling, and any
adverse outcome that I achieve.
